Consent Form
Before receiving a tattoo from the artist you’ll be asked to complete a tattoo consent form in person. This is a legal document required for every session, and it must be filled out in the shop after you've seen your design and before your tattoo begins.
Why It’s Important
The consent form is in place to:
- Confirm that you understand the risks associated with getting a tattoo.
- Collect relevant medical information to ensure both of our safety.
- Verify your age and legal eligibility.
- Provide legal protection for both you and the artist.
What You Need to Know
Forms are completed in the studio, please do not fill in the form beforehand.
Take your time when completing the form, as it is a legally binding document.
